Healthy Alliance is transforming how health care is delivered, shifting focus from singular medical interventions to a more proactive approach that addresses the whole needs of a person on a much larger scale. By addressing health from all angles, Healthy Alliance cultivates the type of change that improves health and empowers the underserved. Its network covers 25 counties across Central New York, the Capital Region and the North Country. It offers approximately 2,350 programs and services to community members with health-related social needs at around 1,300 physical partner locations. 

In the last year, Healthy Alliance added 35 employees as the organization opened offices in Syracuse, Schenectady and Glens Falls. Healthy Alliance added almost 300 new community programs offered by partners in its social care network and helped over 11,000 community members get connected to needed services, like healthy food, transportation, education and employment assistance, and more. Healthy Alliance remains committed to bridging care gaps so that all community members can be healthy, regardless of their circumstances. 

Healthy Alliance recently celebrated five years of being an independent practice association (IPA). As one of the first IPAs in the nation focused explicitly on social drivers of health, Healthy Alliance has an incredibly vast network of diverse providers that work with the organization. The number of partners in Healthy Alliance’s network continues to grow year over year, a testament that the work being done as a collective is moving the needle on population health.
